MEDICAL EXCUSES

Mrs. Strouse is the school nurse at Northern Middle School. Please send all parent note's and doctor's excuses to the nurse for her review. This information and/or the reason for missing Physical Education class is confidential. Northern School District: 432-8961; Ext. 1312 for Mrs. Strouse.

Illness/Injury Notice- 

1. If your child needs to be excused from Physical Education activities, please
write a note, date it, put the reason why the student will be not participating in PE, and have your child bring it to the nurse.  The nurse will write a gym pass for the student to give to the PE teacher.  

2. If a doctor limits Physical Education participation, please ask them to be    specific as to the amount of activity or what your child can/cannot do, as well as when they are able to again fully participate in class.

3. In the event that no statement is received, we will use our discretion in deciding whether the child will participate or not.
